Toolkit - Non-Communicable Diseases Toolkit for Health Professionals, Patients and the Public

This simple educational toolkit informs individuals on positive behaviour and lifestyle changes to avoid chronic or noncommunicable diseases (NCDs). It empowers individuals to work with their health professionals to establish personal health goals and to track their progress.
An updated version of the Health Information Card included in the toolkit was published in 2023.
